Distinguishing Etiology and Documenting It Correctly

At the Bedside, Look for These Patterns

  • Venous ulcers appear between the lower calf and medial malleolus. Shallow, irregular margins, moderate-to-heavy exudate, sloughy base with granulation tissue. Surrounding skin shows hemosiderin staining, edema, and thickening.
  • Arterial ulcers show up at pressure points: toes, heels, lateral malleolus. Deep, punched-out, minimal exudate, necrotic base. Skin is thin, shiny, and cool with diminished pulses.
  • Mixed etiology catches providers off guard. The wound may look venous, but the ABI shows something else. That's why the vascular assessment isn't optional.

Documenting Etiology for Payers

Don't write "leg ulcer" or "chronic wound." Specify the etiology, location, ABI value, and CEAP stage.

Example: "Venous stasis ulcer of left lower extremity, medial malleolus, ABI 1.1, CEAP C6."

For Mixed Etiology, Document Both Components

"Lower extremity ulcer with venous and arterial components. ABI = 0.7 indicating moderate arterial disease. Vascular referral placed [date]. Compression held pending vascular clearance."

How Often Should I Reassess ABI During an Active Treatment Course?
